Emerging from the frustration of patients and in response to the prevailing medical office issues and insurance costs, Four Corners Health Care was established to provide personalized, accessible, and high-quality primary care to the residents of Lake, Geauga, Cuyahoga, and neighboring counties in Northeast Ohio.

By limiting our patient panel, we ensure enhanced accessibility, including same-day or next-day appointments and direct provider access via phone, text, or email. This allows for longer, more thorough visits focused on proactive health management and prevention, ultimately aiming to reduce the need for costly emergency interventions and improve long-term health outcomes.

Our office at 7535 Fredle Dr. Concord Township, OH officially opens on the week of April 6. Patient visits will start on April 13. We are now scheduling for FREE CONSULTATIONS over Zoom and by telephone to see if our services are appropriate and a good fit for you. A limited number of founding members will be accepted during the first month of our services, which will include a lifetime discount on continuous membership, 50% off first-time joining fees and priority scheduling for the entire first year
